DESCRIPTION:Health Trip packets are available after December 5\, 2012. Pick up your packet at any of the following locations:\n\n	\n		Sauk Prairie Community Education Center\, 730 Monroe Street\, Sauk City\n	\n		Sauk Prairie Memorial Hospital Wellspring Wellness Center\, 80 First Street\, Prairie du Sac (use Oak Street entrance)\n	\n		Sauk Prairie Chamber of Commerce\, 421 Water Street\, Suite 105\, Prairie du Sac\n\n\n	Return registration forms to the Community Education Center by January 4\, 2013 along with the $18 fee per member.  (Late registrations will also be taken from January 7-11 at $23 per team member.)\n\n	Kick off Event\n\n	 Tuesday\, January 8\, 2013\n\n	Sauk Praire Schools Community  Education Center\n\n	Reporting Dates and Times\n*If you email hours\, you will receive a response indicating that we received your email. If you don't receive our confirmation\, please contact the office at 643-8386.\n\n\n	1st reporting date: February 11\, 2013      7:30 am - 3:45 pm*\n\n	2nd reporting date: March 18\, 2013          7:30 am - 3:45 pm*\n\n	3rd reporting date: April 29\, 2013              7:30 am- 3:45 pm*\n\n	\n\n	*voicemail available after 3:45 pm\n\n	The team captain is responsible for reporting the team's final total hours to the Community Education Center by May 10\, 2013 (no exceptions). Remember to qualify for the grand prize\, all of your team members must complete the trip and report your hours by the last reporting date (April 29). The grand prize drawing will take place at the Health Trip final even on May 13\, 2013.
